As Captain, I understand that the balance of all contracts will be billed prior to Sept 1, 2008 and that payment is due according to the payment schedule below. Billing will be done from the information given on this confirmation form. Any change of information will be made before August 10, 2008.

I have informed all players in my group that:

• The membership form must be completed and returned to NSTC by 6/1/08.
• NSTC has the right to change court assignments.
• Payment of the contract is the responsibility of all the players and individual billing is done by NSTC as a convenience to the players.
• Interest will accrue and be payable on all balances not paid in a timely manner.
• Contract time may be revoked without refund if players are not members or have not paid a guest fee.
• Nashua Swim and Tennis Club and its staff are not liable for any accidents which occur on the premises and that all players use the facilities at their own risk.
• Players must abide by the 2008-2009 Rules and Regulations. Players who have not received a copy may obtain one online.
